SUBMITTING YOUR ABSTRACT

2. Authors and Affiliations

Any information you enter for authors and affiliations will be used for other electronic products that display the Proceedings:

• Fill out the author names and the affiliations in the spaces provided.

3. Special Audio/Visual Equipment

• Special audio/visual equipment requests must be indicated here or availability cannot be guaranteed. Each room will be supplied with a 35-mm projector, overhead projector, and computer projector.

4. Abstract Title

• Enter the title here

5. Speaking Preference

• Indicate if you would like to be considered for a platform session, poster session or either.

6. Notification

• Presenting authors of all submitted abstracts will be notified by email of abstract status and schedule by January 5, 2006.
• It is the responsibility of the presenting author to inform all co-authors of disposition status (if accepted).
• All notices will be sent as bulk email messages. Many times web-based email accounts (e.g., Hotmail, AOL and Yahoo) automatically send bulk email messages into the junk mail folder. Please check both your inbox and your junk mail folder for your notice.
