Registration Forms

Fees for early registration received by December 15, 2005

Fees for registration received prior to February 15, 2006

Onsite registration fees

The registration fee includes:

  • Access to conference sessions
  • Mid-session refreshments as scheduled in the conference program
  • Abstract book available at the conference and conference documentation

Please note: The registration fee does not include meals on conference days. However, Accommodation packages at Granlibakken include all meals.

Payment
All fees for conference registration are payable in advance of the conference by February 15, 2006. All fees for accommodation are payable in advance of the conference by March 1, 2006. In order to expedite the registration process, please complete the registration form with your credit card details. Confirmation of your credit card payment along with registration confirmation documents will be sent to you. If you do not receive confirmation of your registration 7 days prior to the event, please contact the conference secretariat.

Checks should be made payable to Granlibakken Conference Center and should be sent with your completed registration form.

Cancellations

For Conference Registration
Substitutions may be made at any time but please advise the Conference Secretariat of a change of name. If you find it necessary to cancel the registration completely, please notify the Conference Secretariat immediately. Provided written notice is received by February 15, 2006 a full refund will be given less a USD$50 administration charge. It is regretted for registrations cancelled after February 15, 2006, the full fee (100% cancellation charges) will be payable.

For Accommodations at Granlibakken Confernece Center
Substitutions may be made at any time but please advise the Conference Secretariat of a change of name. If you find it necessary to cancel the registration completely, please notify the Conference Secretariat immediately. Provided written notice is received by February 15, 2006 a full refund will be given less a USD$10 administration charge. Written notice received after February 15, 2006 will be subject to a fee of USD$100 per person. It is regretted for registrations cancelled after March 12, 2006, the full fee (100% cancellation charges) will be payable. No shows at the conference will be charged 2 nights at the package rate an participants who check-in a day late or checkout a day early will be charged the full package rate unless changes are confirmed before March 12, 2006.

Changes
